As a service to prospective applicants who are NOT already attending OSU, the Professional Admissions Office will evaluate previous coursework completed at other institutions to determine if prerequisites for the designated Professional Program have already been completed. You can download the Prerequisite Evaluation Form at: http://professional.osu.edu/profpdf/PrereqEval.pdf. You may complete, print, and mail to: The Ohio State University, Professional Admissions, P.O. Box 182003, Columbus, OH 43218-2003. Official transcripts should be submitted as well.

Most of the programs require all documents to be submitted by the deadline. However, in some cases programs allow flexibility with documents such as transcripts or references. Please note that it is always a good idea to have all required documents submitted by the deadline. In this way, you will ensure that your file will be ready for review in a timely manner.
