health professions, OSU offers seven accredited baccalaureate
degree programs in the School of Allied Medical Professions:
- Physical Therapy is a graduate-level program with information
available at amp.osu.edu/PT/.
- Occupational Therapy is a graduate-level program with information
available at amp.osu.edu/OT/.
HOW TO APPLY
Applications
Please read the instructions in the section below before completing your application.
Choose the application that applies to you:

current or former Ohio State students, or
-
students completing prerequisite courses elsewhere who hope to
be admitted directly to professional study in Allied Medical
Professions at Ohio State.
The Allied Medical Professions application is NOT appropriate for:

those who have never attended college, or
-
transfer students who plan to complete prerequisite courses at
Ohio State
before entering the professional program.

APPLICATIONS & DEADLINES

If you are applying to more than one program (you may apply to as
many as three), a separate application is required for each. The application period
for all programs opens September 1
each year. The application deadlines for each program are listed below.
SELECT APPLICATION
Choose the application that applies to you:

current Ohio State students and former Ohio State students whose last quarter of enrollment
at Ohio State was less than five quarters from the current quarter may complete the
Allied Medical Professions application
on their Self-Service page in the Student Information System (SIS).

non-Ohio State students completing prerequisite courses elsewhere who hope to be admitted directly to
professional study in Allied Medical Professions at Ohio State should complete the following
Allied Medical Professions application.

those who have never attended college, or transfer students who plan to complete prerequisite courses at Ohio State
before entering the professional program should apply at undergrad.osu.edu

ADMISSION CRITERIA
For all Allied Medical Professions
Admission to each of the programs within the School of Allied Medical
Professions is selective. Although the criteria vary by program,
most applicants are evaluated on the basis of:
- Completion of prerequisite courses
- Grades received in prerequisite college courses as well as overall
GPA
- Knowledge of the profession as demonstrated in the personal
statement and the professional experience record
- Interview (if required by the program)
Applicants are encouraged to check their program of interest's website for
specific criteria.

COSTS / FEES / DEPOSITS
Costs
These estimates are meant to provide a guideline for new students enrolling
for 2011-2012. The estimated expenses for the first three quarters
include:

No application fee is required if you are or have ever been an
enrolled undergraduate student at Ohio State.

Only one fee is required even if you apply to multiple Allied Medical
Programs
Acceptance fee (non refundable)
$100 non-refundable acceptance fee is required of new students
to OSU and OSU students who are being admitted to a degree granting
college at OSU for the first time.

ENGLISH LANGUAGE REQUIREMENT
International applicants from non-English speaking countries are
required to fulfill the TOEFL, MELAB or IELTS requirement. If your native
language is not English, you should possess a minimum level of English proficiency
as follows:
